• Care Advocate’s team consists of Licensed Clinical Social Workers, Registered Nurses and, Licensed Social Service Workers.
  Because the Care Advocates team has been composed of professionals who have worked in skilled nursing homes and Assist Living’s and hospital and/or Home Health and Hospice agencies, the Care Advocates team is able to draw from decades of cumulative experiences to benefit the New Choices Waiver applicant.
• When you choose Care Advocates: a Care Advocates team member will call you within 24 hours upon receiving your referral.
  One Care Advocates team strives to meet w/ you and complete your intake assessments within 72 hours!
  The State allows 14-days but the Care Advocates team knows that applying for and qualifying for the Waiver can be stressful and worrisome and so we do all that we can do to make this less stressful.
• At Care Advocates, we strive to “think outside of the box” and we have made it our motto to go above and beyond with every client and family we represent. We do not stop at Case Management because our clients and families often need additional cares, services.
